1.住宿登记（Accommodation Registration）

① 护照原件（Passport）

② 机票、火车票等票据（Flight Tickets, Train Tickets, etc）

③ 要求（Requirements）：需在入境或到达石家庄24小时内持以上材料到主楼406办理手续

The students must take the above documents to Office 406 of the Main Buildingwithin 24hafterenteringChinaor arriving in Shijiazhuang.

2.居留许可（Residence Permit）

① 住宿登记证明（Proof of Accommodation Registration）

② 护照、签证、最新入境记录（Passport, Visa, Recent Record of Entering China）

③ 体检表（Physical Report, 1 copy）

④ 缴费证明（Invoice of Payment）

⑤ 白底二寸照片（Passport photo, 1 piece）

⑥ 相关费用（Fees Charged）：6个月 400 元，12个月 800 元。

（400 Yuan for6 months, 800 Yuan for12 months）

3.居留许可延期（Extension of Residence Permit）

①住宿登记证明（Proof of Accommodation Registration）

②护照、签证、最新入境记录（Passport, Visa, Recent Record of Entering China）

③体检表（Physical Report, 1 copy）

④缴费证明（Invoice of Payment）

⑤白底二寸照片（Passport photo, 1 piece）

⑥相关费用（Fees Charged）：6个月 400 元，12个月 800 元。

（400 Yuan for6 months, 800 Yuan for12 months）

申请办理延期的学生应在居留许可到期30日前将上述材料交到主楼406

Studentswho apply for extension should submit the above documents to Office 406 of the Main building at least 30 days before the expiration date of the residence permit.

4.护照变更(Passport Change)

①在中国申请办理新护照的，收到新护照10个自然日内到出入境部门办理居留许可换发手续，所需材料参照“居留许可延期材料”，并附使馆证明（需要证明新旧护照属于同一人，将于什么时间收到新护照）

Those who apply for a new passportin Chinashall go through the procedures for renewing the residence permit at the Exit and Entry Administration within 10 days of receiving the new passport. Materials needed shall refer to the Residence Permit Extension Materials with the Embassy proof (to prove that both the new and old passport belong to the same person and the time of receiving the new passport)

②回国办理新护照的，来华入境时“入境章”需加盖在新护照上。

Those who apply for a new passport back home should have Entry Stamp on the new passport when enteringChina.

5. 体检（Physical Examination）

① 护照原件（Passport）

②2寸证件照3张（3 pieces of passport photos, 2 inches）

③体检要求（Requirements）：体检前不要吃东西，不要喝水。

No eating and no drinking before physical examination.

④ 体检费用（Examination Fees）：RMB426Yuan/Time

⑤ 办理地点（Address）：河北国际旅行卫生保健中心（石家庄桥西区红旗南大街409-1号）
